• On the application the fourth question is: "Were you referred or given a code by an organization or State Association?" Click Yes. Then another box will open with the question "Please enter the referring organization or code:" Type in CAMPS.

All DMEPOS Medicare providers must obtain this bond. There are very few exceptions and VGM Insurance can help determine if you qualify for an exception.
